I've looked around, apparently it's an offer on a slip in the Ratatouille 2 disk DVD pack.

Claim your free book today - phone 0871 880 8245.

1. Offer open to all UK residents (excluding the Irish Republic) over 18 years of age.
2.Applications must be made by telephone (calls will cost no more than 8p per minute from a BT land line. Calls from mobiles and other networks may vary) Call will last no longer than 3 minutes (mine lasted 1 min 8 secs)
3. Applications must be received by 31st Dec 2008
4. Subject to availability whilst stocks last.
5 Please allow 28 days for delivery.
6 Max of one book per household
7 No cash or other alternative provided.

As the OP says make sure you opt out of anything else and remain firm that all you want is the free book.

Its with Grollier books and they try to offer you 4 disney books and try to get you signed up for their monthly service, I said no thanks to all but the free book!
