Doesn't sound like it if that is all they sent.

Q.What does a cca agreement look like and would it have to be signed by both sides ?

Originally it would have to be signed. A copy supplied under a CCA request can have the signature omitted it seems.

But if it goes to court you can insist they produce a signed one.

    Q. And if a cca is not the first application form, then would I have ever had one ?

Many application forms are not a valid CCA, but there are some that will be.

    Q. The Pescribed Terms are these but do all of the below have to be on or in a special document and signed by both parties or just one?

The prescribed terms should be contained in the signature document.

It doesn't sound as if they have complied yet, if you first statement regarding what you were sent is correct.
