Heres another couple of questions.
on the list of creditors, do i put the collections companies that i pay the dmp money to or should i list the original CC company??
how specific do i need to with start dates for credit agreements?
if they are now via collections company, do i contact these for start dates or the original credit company?

It mentions the bit below only in reference to declaring cards held, but it applies to all debts.
A rough date for the start of an agreement is fine. If the OR/examiner needs any extra info then they can ask the creditor if need be.The address of the bank or supplier should be available from your last statement. Where the debt has been purchased or taken over by another firm provide the name of the original supplier, e.g. Barclaycard, but the name and address of the company which has taken over the debt collection.Free/impartial debt advice: National Debtline | StepChange Debt Charity | Find your local CAB
