i would try emailing them or re signing up as i had mine less then a week after applying for them.
heres the contact us link for tesco http://www.tesco.com/help/contactus/ however it doesnt say baby and toddler so im unsure of which part you would use to contact them by phone but these are the only numbers available as far as i can see.
Just looked on my registration email from the tesco and toddler group they did give me [EMAIL="online@tesco.co.uk"]online@tesco.co.uk[/EMAIL] as the email address to contact them if i had any enquiries.
yes you do have to buy a pack of nappies (mine was a fiver) however i was quite happy with this as they can go in my hospital bag and the basket has more vouchers off inside - did you use your coupons iamjen that came with the voucher for the basket as i had money off and lots of extra points

Def not totally free as you need to buy a pack of nappies, i joined not long ago and got my basket. But be warned-they run out of stock quick and you can buy them full price off the shelf too (which is why maybe they run out quick-people buying them as presents?).
