Company pension meeting...What to ask?

Hi, i am getting asked today if i would like to join the company pension scheme and would like to know what important questions i need to ask to make my mind up, reason being i have never had a company one before.

How much will the company contribute?
How much do I have to contribute to get that company money?
What type of pension is it? Do I get paid an income based on my final salary? Or is the pension based on how much is put in and the performance of the stockmarket?
Can I get the money out of the pension if I leave?
What happens if I need to stop making contributions?Trying to keep it simple...
