missing rebate

we're waiting for a tax rebate that revenue & customs say they paid to our accountant 4 months ago.The accountant said she hasn't recieved it, and that it goes via other people. Is this true? Who should i start taking it up with?

    I cant think who it would go via?

    I would speak to HMRC to confirm what, how and when they sent it.
