i am currently setting up my DMP
yay me!
but on signing the agrrement with them at the bottom of the back page there is the same with repeated twice

Debrag is right. Sometimes Debt Management Plans are in joint names so we have to have both signatures. If your embarking on a sole DMP you only need to sign and date once.
