Each question features companies that we consider to have the best products in that particular area, which is why they have been shortlisted in the annual Moneyfacts Consumer Awards. Now it is your turn to rate their customer service and decide who wins by scoring out of five those providers who you've had dealings with in the last 12 months.

There are four surveys and it should only take you about 10-15 minutes to complete all four. You can complete just one or two surveys if you prefer and skip questions that are not relevant. Each completed survey gives you one entry into our prize draw, with the chance to win a £1,000 cash prize – so completing all four surveys will give you four entries into the draw.

Tip: The more you answer "no" or "none of the above" the quicker it is.
