Requesting part time hours after maternity leave

I am currently on Maternity Leave and I'm due to return in April 2008.
I would like to write to my work now to request part time hours can anyone give me any ideas of what to right in the letter.
Do you have to included a reason of and example of how you'd like to return I.e 18 hours, 21 or do u leave it for them to decide?

Its nearly three years since I was on maternity leave and due to go back to work however this is part of the letter I sent:
"Dear ....
As you are aware I am due to return from maternity leave on ...... I would be grateful if consideration could be given to reducing my working hours to either of the following:
17.5 hours worked over two and a half consecutive days
Or
21 hours worked over three consecutive days
I would welcome the opportunity to discuss my request further and I look forward to hearing from you in the near future in order that I can finalize childcare arrangements".
I think you need to give them some time to look into it. My employer got in touch with me and offered to reduce my hours (but also make someone redundant) or give me an alternative job. In the end I took redundancy and stayed at home for another 12 months. I only went back to work in January 07 and I'm now about to apply for a different job.

How do I make a request?
You must apply to your employer in writing, stating that you are making a formal request for flexible working under the law. You can make your application in a letter or email or fax, or by using the standard flexible working application form FW (A) provided by the DTI. Alternatively, your organisation may have its own form for you to use – check with your manager.0
