    There is one thing you need to check here. It might not apply, but it's worthwhile checking.

    It's possible that your employer has changed their process to tax you on the medical insurance benefit through payroll. Some employers are moving to that method, and is the HMRC preferred method.

    When a benefit is "payrolled", you would see a line on your payslip for the benefit monthly amount. It would be added to your gross taxable amount for calculating tax, while not being included in the pay.

    If you do not see it on your payslips, then the method of reporting via P11D will apply, and it's normal for that to lead to some further tax being due after the tax year end. Employers are allowed to report P11D data to HMRC by about July after the tax year end.

    nothing on my payslip and low and behold I found a P11D which I had totally forgotten about , it says £1130 medical insurance , it was issue in July for 24/25
