Help with SA101 Chargeable Event Certificate

I've received a letter from HMRC that i must complete form SA101 ( paper not online ) on behalf of my Dad who past away. The request is due to a chargeable event certificate from his life insurance policy . My confusion is which box i need put the gain amount in, its either

Box 4: Tax treated AS paid 
or
Box 6: Tax treated NOT as paid

The gain was £19,279,24
Notional Tax £3,856

Dad was a basic rate tax payer and with his wages , pensions and this gain it still placed him in the lower tax bracket

Any help would be much appreciated and also a heads up if you think there will be tax on this insurance policy that i will have to pay. 

Please also note that no inheritance tax was paid on the estate as it fell below the threshold

    From what you've said it sounds very much like it should be box 4. Does the paperwork from the insurance company not explain that tax at basic rate is treated as having been paid. I was in the same situation a few years back and the paperwork I received made it clear.

    Thank you so much for your reply. I'm leaning more and more towards it being box 4 "treated as paid ". 
    I only received the certificate page from the insurance company via the probable solicitors which only showed the gain, number of years and notional tax . Spoke to the insurance company on 2 different occasions and got advised on putting it treated as paid and not treated as paid but they did confirm they had paid the notional tax amount. Maybe some insurers list it as treated as paid while others like mine mark it as notional tax 🤔
