Marriage Allowance Claim Rejected

I've just had my third marriage allowance claim rejected, the reason given this time is "out of date version of form". This was the only form available to download on the HMRC website at the time of application and is no different to the one they have now posted back to me, bar the date on the footer of the form. My first claim was rejected due to "insufficient information" and contained a request for my husband to submit a letter confirming his name and address. Today I've also received a reply to this letter stating our letter will be returned and they now require a copy of our marriage certificate. I stupidly made an error on the second allowance claim form so submitted the a corrected third form even before the second was rejected. All three forms were downloaded directly from the website and neither of the first two were rejected for being out of date versions. Does anyone know exactly how to claim marriage allowance as it's hard to predict what paperwork will be requested every 4 weeks and I'm now 12 weeks into this debacle?

The out of date form thing has come up before, they updated the form on line but failed to change the date in the footer, usual HMRC incompetence.1
