Hi, I have been paying my debt in debt management plan for over a decade. I have adhd and autism and now lost job and have long term sick mobility issues. My mental health is at an all
time low and I just can’t do it anymore. I am ashamed to say I have considered ending things but have been referred to a social advocate at my GP for support. I have psychiatrist support as well. Is there any way I can get some advice on the dhmef forms / success rate / tips / useful templates - I already have the dhmef form. Thanks in advance for reading

Get the DMHF signed by the psychiatrist (they can't charge for this according to the British Medical Association).
If you are with a debt advice agency give them a copy of the signed DMHF to send to all of your creditors and see if they will write things off.
Meanwhile see if they can assist with getting you a mental health breathing space - this would mean that no creditors can contact you while it's in place nor take any action against you and not charge any fees or interest. This should last for the full time you are in a crisis and then 60 days beyond. No payments need be made during this time either. Hopefully this will give you literally the breathing space you need to get your life back on a better track and then you can look at how to proceed with things.I’m a Forum Ambassador and I support the Forum Team on Debt Free Wannabe, Old Style Money Saving and Pensions boards. Tell your creditors what you have told us and ask them to write the rest of the debt off
If you can send a dmhef with the requests so much the better.
