Optical prescription help

This isn't exactly moneysaving by itself, but I know we have a couple of knowledgeable optical people here. Would anybody please be able to tell what the bit I've circled says and means please?

Hopefully it's big enough to read without being absolutely huge.
(And the BVD, is that "BVD - 5mm" or "BVD -5mm"?)

I’m am not a glasses expert, but surely BVD = minus 5mm otherwise that would be easily subject to confusion.Back Vertex Distance.Is the other part a signature?0

No idea, looks like znoaks but I haven't a scoobie what it might mean. A 5mm BVD seems very small, that is usually around 12mm as it's the distance of the lense from your eye in the test frame/ machine when your eye test was done.Why not call optician that did your test to explain?0
