There is clearly something badly amiss if you have been threatened with demotion and a £10K pay cut. Why not opt for the more emollient (any thus more likely to secure a decent reference) 'personality clash', given your manager's admission you were the wrong hire for this job?

If you are leaving do not put anything about them committing fraud because they are going to get you for this. They are going to sue you for damages and say that you did this without their asking and you will not be able to prove that they asked you to do this. Just let the reasoning be something vague and generic. If you put health reasons they are going to send this as part of your reference and your future employer will think that you are too sick to work for them.0

If you haven't already tendered your resignation then please don't include any reference to the data falsification.Just stick to what they need re resigning and leave it at that.0

The fake Pat Testing seems a little odd as there is actually no law requiring it or even labelling. All that is actually required is ensuring that any electrical equipment is safe or maintained.1

@superbigal - This was for a large external exhibition, the organisers sent a risk assessment questionnaire and specifically asked for all equipment to be PAT tested. They wanted to see valid test records for everything.0

I don't think this is a hill worth dying on. I'm sure the person I knew was capable of inspecting equipment and not using anything with an obviously unsafe plug on it. Indeed, so many plugs now are moulded on pre-purchase that inspection is quite hard.
