After/during an extended period of sickness absence it's pretty much standard practice in larger organisations for contact to be made. I'm surprised it's taken 3 months if I'm honest. The meeting will be to discuss the situation, look for anything they can do to help, long term prognosis etc. If you are in a job which can be done from home they may even suggest that.2

Will depend on your sickness record, your workplace polices etc.
Indefinite sickness isn't sustainable for any organisation, especially if after 3 months you still don't have a diagnosis.
They will need to make necessary arrangements for your workload at the very least as it's not reasonable for everyone else to pick that up on top of their day job
You should be honest. At this stage I expect it's a fact finding meeting to understand exactly what is going on and why you are not in work. Can you really not do anything? Not even light duties? Will of course depend on your role but maybe something to consider.2 -
Have you considered paying for a scan privately? Often tests and consultations are quite cheap e.g. £200 and can advance your place in the queue within the NHS.
if you can’t afford it then obviously it’s not an option, but for someone in a lot of pain I would have thought it’s worth considering assuming there’s not private health cover with your job?1 -
