Hope the following link is helpful:

Could she have been put off, as it may well used to have been done by filling in a paper form.

I presume she has business insurance for her car.

The NHS is run on the good will of employees like your wife, but if you read this forum regularly you will see most people have no qualms about avoiding paying tax.
Thanks, yes she definitely doesn't like the idea of bothering with the car miles but I will try to encourage her to record it, especially as she may doing more domicilliary visits soon when her job description changes.

She doesn't have business insurance for her car, it's honestly never something either of us have even considered. I guess we should look into that.

    Most definitely.  Urgent.
If she has an accident can she tolerate the costs and legal consequences of driving uninsured?

I'm surprised that your wife has never had to produce evidence of her business insurance for her car, as I did, on appointment to a community position (NHS) and every year thereafter whilst employed there.

I even had business insurance when I was on placement in the community as a student.

    exactly

when i worked for the ambulance service ( and  could be sent to different stations for shifts )  I had to produce evidence of  correct  insurance  before claiming mileage   ( had to track down my  station manager  while i was still in training school to do it as well  as we had a scenario day  at the other wend of the service area  and no service vehicles available as there were driving courses on for other cohorts at the tim )

Since the OP's wife has never claimed mileage from her employer the matter won't have arisen.
