Help with form to request statutory demand to be set aside

Hello, I have downloaded the form 1AA to request to have SD to be set aside as husband is disputing amount of debt owed - I am usually pretty good at completing forms and have downloaded the advice form too, but am having doubts I have done it correctly - could any one provide some basic extra guidance or an example of one - husband is personal gurantor for a business debt business is now in liquidation - it asks for case numner on form but there does ot appear to be one of the letter from creditor x Thanks in advice if anyone could signpost be to a completed example that would be a help I think

in particular it is tge last couple of sections - - "insert names/addresses of those on whom application is to be served (if any) and "insert names and address of the persons to whom it is intended to deliver notice on this application are: " would that be the first one would that be Husbands Name (or leave blank) and the second part is that the creditor solicitor details?0
