On my way to freedom! £16k debt free wannabe

It has taken me years to get to this point, but here I am, hello.

After researching on here all the people who have managed their own Debt Management Plans (DMP), this is the route I have decided to take. I was previously on a DMP, then token payments with Step Change. I have now cancelled this and am due to write to all my creditors to let them know I am managing it myself from now on (if I can find all their addresses and details!).

My debt

1. Sold to Moorcroft from credit card £6,050

2. Sold to PRA from credit card £5,390

3. Sold to PRA from credit card £2,135

4. Sold to Moorcroft from credit card £1,195

5. First Direct overdraft £877 paid £350 this month now £537

6. Starling overdraft £345

7. Flint Bishop sold from over-paid salary £477

8. Possibly Octopus from a huge electricity bill, it was passed to someone else, they didn't pick up the payments made from Step Change, was passed back to Octopus, now I have no idea where it's at so i'm leaving this one from now until they contact me... it's around £600.

This is my Statement of Affairs (SOA) 

Income
£1350 a month across two paid jobs (one is a zero hour contract and will reduce slightly in summer, which I will look to bring back up again with other work).

Outgoings
Rent £400
Wifi £46
Fuel for fire £50 
Mobile phone £15
Groceries (incl all toiletries and sundries, cat litter etc and things like parking, the odd coffee or lunch with a friend) £220
Clothing £50
Petrol £195 
Car tax £17.50
Car Insurance £26
Car maintenance £43
Dentist and Psychotherapy £60
Presents £20
Hair £5
Cat food £62

Total outgoings: £1209.50

Income minus outgoings = £143.50

The income is variable as one of my jobs is zero hours, but outgoings are also variable such as wood for fire for example, depends on the season/weather.

Any money I get for birthdays, Christmas, or from my grandma stuffing notes into my pocket ignoring my pleas "No your pint of milk did not cost me £10" etc I am keeping towards an emergency fund. No pet insurance. I recently had a birthday, so now I have the grand sum of £20 savings hurrah!

My plan is to pay my debts off in this order:
A. First direct over draft
B. Starling over draft
C. Flint Bishop from salary over-payment (do they take full and final settlements?)
D. Then start with offering full and final settlements to remaining creditors from smallest amount to largest as and when I have saved. 

Offering token payments of £1 a month to each of my creditors in the meantime. 

If you have any suggestions/tweaks, please let me know.

I'd love to know what everyone thinks of all of this on first glance, and thank you so so much for reading! I'd love feedback and suggestions.
