You send your witness statement to the court and the claimant (or their solicitor) and the judge should read it before the hearing.  I wouldn't mention that the driver has poor eyesight!
You were shown some examples of recent witness statements that include Chan & Akande; have you read them and are you going to incorporate your narrative into one of those examples?

@aza123 includes Chan & Akande

@Kpmp54 is recent & has Chan & Akande and also Smith, Edward & Wilkinson
Is the solicitor DCBLegal or are CE doing it themselves?

Yes I saw that post by C-m previously but it does not state "and also Smith, Edward & Wilkinson" which you have added so I assumed you have seen the actual WS of the named OP  -  just trying to clear up any confusion.

No, it comes from another post from C-m where she advised of some suitable WSs, I've adjusted my standard response to: -
