Missing vaccine records?

I have no idea if this is the right forum to put this in but any help I can get would be appreciated.
My mother died earlier this year.
Her health declined quite badly after having her COVID vaccines (I am not anti vax and this is not a thread against the covid vaccine).
I have letters from her consultants that stated she had a reaction to the vaccine which worsened her health.
After she died I decided I wanted to investigate the covid damage payment scheme.
I applied for access to her medical records.
I spoke to her GP and it seems both her covid vaccine records are missing.
I have been chasing this up with the practice manager for months - I am waiting for a call back but if she still can't find the records this leaves me very frustrated.
Where do I go to investigate/ complain about this.
I do not understand how TWO of her vaccines can be missing. We know she had them.
My heart is broken and I just want to get this information.

Sorry for the loss of your Mother,
My Husband and I both have incomplete covid vaccination records, this was due to a change of venue on the day. Our Doctor surgery moaned at us for not attending, whilst our Dr did not have record for us attending or having had that vaccine, my record on the NHS app is in fact correct. I don't know how this happened but did your Mum ever have the printed records sent to her or the app?Make £2023 in 2023 (#36) £3479.30/£2023

I just looked online and the NHS website states:Complaining about NHS services
Everyone who provides an NHS service in England must have their own complaints procedure.
You can often find information in waiting rooms, at reception, on the service provider's website, or by asking a member of staff.
You can either complain to the NHS service provider directly (such as a GP, dentist surgery or hospital) or to the commissioner of the services, which is the body that pays for the NHS services you use. You cannot apply to both.1 -
Sorry for your loss. The vaccine damage scheme is long and arduous and you have to be able to prove it was this that caused the death. This will get dragged on for years and will drain you down. Have you had bereavement counselling?0
