P45 details incorrect

My ex employer has issued a P45 and the personal details are wrong. Date of birth has been entered as month/day/ year instead of day/month/year and my surname has been spelled incorrectly. It is double barrelled and instead of Smith-Jones they have put Jones-Smith. The NI number and address are correct
I have asked them to reissue it but if they don't what are the practical implications?
