Don't bother calling the JobCentre (or HMRC).
You need to complete a form JSA NC1 and deliver it to the JobCentre - in person or by post to your local centre.
The JobCentre can give you the form in person but doesn't make it available for download or by email. However, Google knows where to find copies requested by industrious FoI requesters.
