LGPS spouse pension and ARC increases - calculation errors

Yes, I am quite sure that I opted for the ARCs with spouses benefits. 

I kept copies of everything, including a copy of the relevant table extracted from the 2008 GAD regulations that I sent with the application form, with the amount per £250 circled, and a simple calculation showing the multiples of that figure to be subtracted from my salary each month on the application form.

I also have original payslips to submit as proof if necessary.
