Transferring a pension to SIPP

Hi,
My sister has a couple of pensions that are quite old. These were set up through various local brokers (FAs and IFAs) whom have merged, been bought over etc. a few times.
She wishes to consolidate these. The biggest pension contains two investments - both with Scottish Widows. She has the SW account numbers for these.
When transferring, does she enter these SW account numbers and the gaining pension platform works directly with SW to sort the transfer? Or does she need to make any reference to the agents? Can she just avoid anything to do with the agent. She did contact the current agent and they were less than helpful in giving out information (including basic information such as the fees she was paying).
