Have a look at her increase letter from around March this year.
Is a Contracted Out Deduction shown? Assuming that she is not a widow to whom this only applied after becoming the beneficiary of a late spouse's pension, this would indicate that at some point she was a member of a contracted out pension scheme.
If she worked in hospitals, there is a chance that she was a member of the NHS scheme.she applied for her record from HMRC last year0 -

It's more a case that if there is a COD, then she was a member of a Contracted Out Scheme at some point before April 5 1997.
That said, mistakes happen and there has been a catalogue of errors and omissions in DWP systems.
After that date a different arrangement relating to contracting out.
She will likely have some Graduated Pension.
What is shown on her letter by way of Additional State Pension?
If she was employed and earning between the late sixties and up to her state pension age (2012/13), and not contracted out between 1978 and SPA she should have a reasonable amount of SERPS/S2P (Additional State Pension).

Kste1 Not wishing to be too discouraging, but unfortunately the NHS records of old pensions are not always complete - I seem to remember someone mentioning this on a thread here, and certainly my mother was never able to track down her hospital pension even when some pay slips confirming payments to the scheme were found.You will need to provide as much information as possible in terms of dates, locations, and any official documents she may have retained.

I don't think you have lost your pension - just lost track of it. See if Gretel.co.uk throws up anything useful if you try searching via that.
