Current 'Prove it' letter template

Hi all, I can see a link from 2021 to a 'prove it' debt letter template - can anyone provide a link please for the most current 'prove it' debt letter template? 
I am struggling to know what to put in the letter.  I need to ask for proof of a debt as I have recently discovered a CCJ (2019) for a student loan debt (the student loan date quoted is 1993).  Having spoken to a debt advice place (free local one) they advised I send a prove the debt type letter to the Erudio Student Loans - the creditor.

    The regulations have not changed significantly since 2021, and our template letter covers most of the bases, its up to you to edit the letter to your personal circumstances.

    As legal action has been taken, that pretty much means the creditor does not need to respond to such a letter as a court has deemed you liable.

    What you should be asking for are details of the judgement from the court, and then asking the creditor why they have taken such action, they no longer have to prove anything to you, your only option would be to apply for a set aside if you feel its not justified.

    "Provit letters" are only suitable on debts that have not seen any legal action.
