Settlement Template Letter / DMHEF

I have seen some users have contacted their creditors for a reduced settlement amount. Does anyone please have a template letter that can be used for this? (I'm not currently in a position to do this yet, but it would be helpful to have the letter saved for if and when I am in a situation to settle my debt.) I have a DMP with Stepchange - I think I have read that I would need to contact my creditors directly rather than through Stepchange?
I am also looking into getting a healthcare professional to fill in a Debt and Mental Health Evidence Form. Under the Advice/creditor details section, would the Stepchange details be entered here, or would the healthcare professional need to complete the form for each creditor? Also is this something Stepchange could send to the creditors on my behalf, or would I need to send this directly?

Do the DMHEF form yourself, and first. See if you can get any of them to agree to write-off/ not pursue

On the bit of the form where it asks for Advice / creditor organisation, which advice organisations could be used?
I hoped I could put Stepchange here, as the healthcare professional may not be prepared to fill in the form each time for each debtor.0 -
I don't see why stepchange wouldn't support you. You could contact them. I think they have webchat.
Other organisations would be Citizens Advice, Payplan, National Debtline, Christians against Poverty, Community Money Advice0
