Occupier(s) Consent Form - Nationwide

I have a question around the Occupier’s Consent Form on the Mortgage Deed for Nationwide.
One of the conditions of the mortgage is that the named occupiers (4 people) sign the form.
One of the conditions of the mortgage is that the named occupiers (4 people) sign the form.
My question is how does that work when the form only has space for two signatures? There’s only space for two witness signatures too.
