Call from my IPA firm

Hello all.
I've had a call from the firm that deals with my IPA, they have asked me to send an updated form even though I sent an updated form a couple of months ago, I rang back and asked why it was so soon to re-evaluate as it's supposed to be done every 6 months, they told me the last one I sent there was only 1 page filled in but I've went back and looked at my outgoing mail and it had been filled in correctly, I find it strange it's took so long for them to get back to me about the so called issue of it not being filled in properly. Do I need to contact my OR about this?

Re-send the previous one and say there are no changes? (If true)

You'd hope this wouldn't happen but I have known email attachments to be printed out double sided, then scanned back in single sided.

Yeah still going to send it over, I digitally typed onto the document and screenshot it so something isn't right. Probs cos minimum wage has just gone up is why they're asking now.

A lot of things do get a small hike at the start of April so it does make sense to do an annual review at that point.
