Leaving NHS with less than 2 years service

So, as I predicted a job I have with the NHS may be ending before my 2 years service is completed. 
Sadly, I didn't manage to get another pension pot transferred into the NHS so I will have three options if this happens. 

Can anyone elaborate on this option please? 

Option 2 Transfer the value of your benefits to another registered pension scheme subject to satisfying time limits. If under Normal Pension Age, and you joined your new pension arrangement within 12 months; and you apply for the transfer within 12 months of joining your new arrangement

So, it cannot be transferred to an existing DC pension pot? 
Would it be the full transfer value of my contributions, plus employers plus tax relief or just my contributions?
