As above, contact Age UK for expert assistance.
Here is their Factsheet on Pension Credit.

My OH’s deferred pension was calculated incorrectly.
I only realised when I found a statement lying around. It took some time to get corrected.
These things do happen and it’s worth persisting.2 -
Just thought I'd add this update here. The old gent passed away in May. In penury. No reply from the DWP to the numerous letters they sent, not even to the Pension Credit application. His widow has Alzheimer's and a serious heart condition and is in mourning, no shape to chase up on anything, i.e., not even to remember details for forms. Though 50% of the time she does have full clarity. Very very sad.0

Presumably the DWP has been advised of the death.
Has the widow not received anything at all from DWP?
Could you contact Age UK on this lady's behalf?

Yes, DWP have been informed. I explained the issue to Age UK and passed the contact details. No idea what happened after that. But nothing from DWP so far is what I've been told.1

Just thought I would leave a status update here. The widow has now received her late husband's outstanding arrears: Appx £42k. This was in addition to the £9679.74 DWP sent him in Nov 2021. Case closed. Thank you for all your help.

DWP gave no explanation for the miscalculation, no explanation why the gent wasn't sent the standard form offering the choice whether to take the arrears as an increased weekly payment or as a lump sum. No explanation.
Age UK were of no use as the couple moved back to Pakistan in April 2023. They only help resident OAPs.2
