Tax, Interest and Penalty calculator

Dear all,
I have rented a house for a few years and I have not paid taxes for the rental income. I am trying to fix this by disclosing the earnings to the Inland revenue using the Tax Interest and Penalty calculator in their website. My problem is that I do not understand what they are asking me for In the form, I get a line with 5 boxes to complete fro every year: Description, Income Tax due, Class 4 NIC due, Capital Gains Tax due and Penalty rate applicable. Can anyone advise on what do I need to put in these boxes?
