Got VAT surcharge notice, how to write a letter for review?

The Boss was busy, and I was new to this.
the payment was 5 days late than the deadline, checked bank statement, I just realized the company VAT payment was late last 12 months, this might be the 3rd time.
how to write a letter for review, is it still possible to get the surcharge cancelled?

You may want to include in your letter the reasons for the other late payments as HMRC will have issued notices for each of those warning of the consequences of further late payments.
