I'm afraid I don't know the answer to that one either. Debt doctor's post seems to make a lot of sense, but something tells me it might not be quite so easy.
I think professional advice is called for; either a solicitor or insolvency practitioner. There is an I.P called Brin on the Debt Help UK Forum who might have the answer.

I dont think I can help much I am afraid. We get all our bankrupts to sign a Tax and National Insurance Disclosure Authority early on in the bankruptcy proceedings. I do not know why a Insolvency Practitioner would need one signed after discharge.0

Just had a thought. It might be as simple as the Trustee wants to check wheterh you paid any income tax after the bankruptcy order, when you ought to have been benefitting from a Nil Tax code. If you paid any income tax after the BO date until next financial year, the Trustee will get a refund paid to him.
I would honestly just sign it. Failing to return the form as requested may ring alarm bells for them, and make them investigate your income further than they otherwise might have.0
