Self assessment

First time posing here.  Sorry if any mistakes. 
I am doing my first ever self assessment as a medics.  Basically I have a monthly income from a fix-term job. If I work extra shifts, I get paid weekly and at a higher hourly rate.  My employer is the same.  I get separate p60s. On the self assessment, do I put my employer as one and combine all income or as separate jobs?  Another question is regarding NIC.  Is it regarded as 1 job or 2 jobs - if working at 2 different pay rates (same employer) in terms of NIC?  Many thanks

    You need to ask your employer about the NIC, I think it could be two separate jobs for NI purposes or it could be combined
