    That’s absolutely what happened. I have asked for templates for various documents before and all that is available is a previous doc that I then have to modify to suit my purpose (this is for operational non HR stuff).

    Yes, they've either no updated it or there are two alternatives depending on the nature of the role and they forgot to delete, 

    However, given the offer letter I think in your case the 12 weeks applies

    My opinion (and we can only give those here) is that given this is a clear error by your employer who drafted this mess, that the outcome should be the most favourable one to you.

    So if he wants 4 weeks' notice, insist on it.

    However it could end up with m'learned honour to rule on it given all the evidence.
