FATCA letter from bank

My bank has sent me a letter asking me to complete a FATCA form, it’s an IRS W9.
I live and work in the U.K. all my life and have no USA bank accounts. I am dual National though due to my mother being born in states.
the W9 form asks for name as it appears on income tax form ( which I do not have as have never got a tax form from America) and then address and business info etc - not really sure what to do and local bank have been not helpful

Assuming you have a valid claim to US citizenship, but only if, most folks would use the official penalty-free IRS Streamlined Foreign Offshore Procedures (SFOP) as the lowest risk and lowest cost way of catching up. https://www.irs.gov/individuals/international-taxpayers/u-s-taxpayers-residing-outside-the-united-states
