Amending a risk assessment after installing something that makes equipment safer is not going to be top priority. The important thing is someone assessed it and measures were put in place to ensure it can be used safely. The paperwork will follow on.

Agreed. However, it can't be used until a risk assessment is completed.

Update. My friend's boss has directed him to reset the tool to its original state. The people involved in it's modification were not willing to produce a risk assessment. A good outcome from his perspective.
