It sounds as though this is part of an investigation and may lead to disciplinary proceedings depending on the outcome.
I've been involved in these as a manager (although we would term it an investigation not an informal meeting) - sometimes they lead on to disciplinary action, sometimes not. The last one i did we started out with 4 things we were concerned about, and the in the investigatory meeting the employee was able to provide explanations for two of them. We did have to move forward to disciplinary action regarding the other two, but one of the ones which we didn't proceed with would have been the most serious (It turned out that the person had failed to record something properly, but had done. Not doing it would have been gross misconduct, not recording it was a much less serious issue)
