Report and repay the loan charge- BUT I HAVE NOT EVADED TAX?!

Hello all,
The letter says that I have to complete a tax return from April 18-19. I have just looked through all the payslips from the agency I used to work for and the bank statements. They all match and there does not seem to be any dodgy activity so it seems that I HAVE indeed paid all of my taxes and not evading.
When I e-mailed the agency, it seems that it's a common thing for third part umbrella companies ( they ones who paid my salary) to evade tax, in fact, after trying to google one, they don't seem to exist anymore?
My question is, now that I have looked through payslips and bank statements, what are the next steps to ensure I do not get a fine. I believe I have to fill out the

Has this happened to anyone before, I have actually never filled out a form like this before so I am learning as I go, I just don't want to get it wrong or get a fine. The HMRC has not been able to help me ( they tell me to go on their website but I can't seem to understand the jargon on there).Many thanks,
