Pension benefits

Hi I am shortly taking voluntary redundancy. I have been asked to fill in a series of forms relating to my final salary scheme which was finished a couple of years ago, and I had to join a money purchase scheme. My final salary scheme was fully paid up. I have only paid a fixed percentage of my wages into the scheme from my salary and no avc`s.
I have have sent a picture with this link, and my question is, do I just need to tick the no box in every section. I joined the final salary scheme in 1973 and stayed in it until it finished in I think 2018.
Any advice is appreciated.

    You say that you joined the FS Scheme in 1973.  When do you expect to draw the pension?
    Is there a retirement date shown at the top of the letter?

    It's unclear if you need to tick yes on the third box down and complete part E - what is the retirement date shown at the top of the page as mentioned in this box down ?

    Tick the third box with a 'yes' and complete Part E.

    If you don't have a protection certification, that too is a 'no' and you then just sign and date the declaration.
