I am stuck on a question in a application form

On a application form i am currently doing in AWR Work History Declaration section it says "I can confirm that during the previous 8 weeks the full list of companies (hirers and connected hirers) I have worked at via any and all different contract structures are:"
In the previous 8 weeks i worked at Rico Logistics TVS SCS and i still work there but it is a zero hours contract. Should i put that in the answer box

They are asking where you have worked in the last 8 weeks, regardless of contract,
You put this information on the application form.
Simples.

What, that it's a zero hours contract? No, because that's not what the question asks.
