I am stuck on a question in a application form

On a application form i am currently doing in AWR Work History Declaration section it says "I can confirm that during the previous 8 weeks the full list of companies (hirers and connected hirers) I have worked at via any and all different contract structures are:"

In the previous 8 weeks i worked at Rico Logistics TVS SCS and i still work there but it is a zero hours contract. Should i put that in the answer box

They are asking where you have worked in the last 8 weeks, regardless of contract,
You put this information on the application form.

What, that it's a zero hours contract? No, because that's not what the question asks. They just want the names of the company or companies where you've worked, plus (if you were sent by an agency) the name of the agency.
