Hitachi early settlement nightmare

Hi everyone, wondering if someone could kindly explain this letter I've received from Hitachi over the weekend. In 2018 I took out a ridiculous high interest loan in desperation. I was lucky enough in May of this year to be able to settle the loan agreement early. I had £9793.68 left to pay but a settlement figure of £6160.91 paid in May seemed to finish the loan and bring my balance to £0. I had correspondence confirming this (a letter I can't find!) and even my credit file profiles have listed the account as settled and the balance as 0. 3 months later I receive the letter I've attached as an image, stating a new outstanding balance of 3381.65. I've logged on to my Hitachi account and there it is too! And apparently a direct debit of £251.12 will be taken from my account on August 23rd despite me cancelling it after settling the loan.
Is there something I'm missing here or are they trying to pull a fast one.... Any help with this would be much appreciated!

    On the face of it the quoted c£6K on a c32% APR loan was the correct settlement amount after 2 years and somewhere along the way something has fouled up and it has not been allocated as a closing payment.  A computer generated letter because a correct box wasn't ticked at the time so a phone call /letter should clear it up.
