New Tenant - no fixed address

I have a small bedsit and I want to rent it at a low cost to a friend of a friend who has hit hard times - family breakup and divorce.
Initially I was happy to let him stay without rent but then I was advised to formalise the agreement and charge some rent.
He has been living with friends, extended family etc. Now he is working and I have confirmed this.
So looking at the online form for my council it asks for a previous address, so which address do I add?
Do I need permission from whoever I add to the form?
