how to type a letter

Hello Forum. 
I need to cancel my job seekers allowance but the lines are rubbish and after some hours i decided to issue a letter directly to the center. 
I am looking for advice on how to style it and what information should be present in it. I am certain of some aspects like nino and my own name but not on the overall view. 
Any help would be welcomed.

    All you need to do is send a letter, including your NINO, plus name and address, saying you wish to stop your claim for JSA from x date.  No need for anything fancy.  You don't need to give any reason for cancellation.

    Date of letter.
    Maybe type in the letter the address you are sending it to, in case of any doubt

    To whom it may concern,
    Your name, address, national insurance number

    I am writing to inform you that I wish to end my JSA claim as of ....... (give date it ends). You can give a reason if you wish, such as starting a new job. 
    That should pretty much cover it.
