So I’m a doctor on £43k. I’m in annual leave the next week and as you can imagine there are loads of extra shifts going. If I earn enough to go into the 50k tax bracket, how will that affect my deductions? Will I just get extra taxes on income above 50k? The following month if I just work my regular shifts, will I get taxed the same as before?

Assuming you have no other taxable income and your tax code is 1250L then yes you will just pay 40% tax on the earnings over £50k.

If the extra earnings are paid at the end of April (month 1 of the new tax year) higher rate tax will be payable on taxable earnings over £4166.66  At the end of May it will be £8333 etc.  So if you pay higher rate tax in April it may all be "refunded" in your May pay if your taxable wages go back to normal levels.
