Virgin Media old debt - default date vs delinquent date

HI all,
I have 3 old debts showing on my credit file from Virgin media
2 of them have default dates so I assume they will fall off pretty soon as the default date is almost 6 years ago ( due to moving house and they saying I didn't inform them) although I had their services at my new address!!
the one that is bothering me is an older one - I will give a small history. I applied for a mobile phone in April 2012, when the phone arrived I had changed my mind and returned it to the local store in town.
a few months later I received a letter saying I was behind with my payments, after several calls they said they had never received this back in store and that I would be liable for it. I asked them to check the phone and see if it had been used etc to no avail.
my issue is , they do not have a default date on my file just keep updating with a delinquent date ...each current month. Surely they cant do this as this debt is over the 6 years and should've fallen off by now.
I have tried calling but to be told they no longer have the details so cant help me. I know I have to write to them and get it cleared. Do they have to clear it, as if the default date range is 3-6 months of non payment then the default date should be around sep/oct 2012. This is obviously was past the 6 years.
Are there any template letters I can use , and who/where do I address this to please.
