Double scan for item at asda

Is this possible? I did some shopping at asda and noticed I am paying for an item twice. It was a small amount and did not bother to go back in store for a refund. I am just wondering how did this happen? Thanks

    Can happen at self scan with scanners in vertical as well as horizontal plane. As you remove item to basket it may catch the bar code again.

    At my self scan it shows in window the items scanned so it's easy to check through before payment, a double scan will show up, as will incorrect pricing
