Statement of affairs

Hi, is there a template for a statement of affairs anywhere?
I started last night but don’t want to miss anything out!
Also the reason I’m doing one is we had a CCA request with Link, they’ve now sent the documents so we are about to start regular payments again. Is is worth offering a a settlement at this stage or are they more likely to decline in favour of regular payments (I think about £33 a month on a £2300 debt). We have saved some money since we haven’t been paying but would prefer to use it to clear another two debt.
